Abstract

Key Points and Objectives of the Lecture:
This lecture addresses clinical challenges in aligner orthodontic therapy and presents evidence-based strategies from five clinical investigations covering patient experience, biomechanics, and interdisciplinary care.
A prospective study of 23 patients showed initial impairment in oral health-related quality of life (OHRQoL) and anxiety, which significantly improved by the retention phase. Comparative data from aligner and lingual appliances indicated transient speech disturbances, while microbiota analysis in 40 patients showed no major shifts except for increased Aggregatibacter actinomycetemcomitans in aligner users.
Clinical cases explored biomechanical approaches to open bite correction, including posterior intrusion and anterior extrusion, as well as a hybrid approach involving fixed appliances, aligners, and orthognathic surgery. A separate case illustrated vertical control using low cost 2D lingual attachments with aligners.
These findings emphasize the need for personalized treatment plans, hybrid solutions, and patient education to enhance predictability
